ADMISSION INSTRUCTIONS

Parents are to make available the following documents when reporting to the school

1. Placement form
2. ACTIVE NHIS CARD
3. Admission Letter
4. Birth Certificate/Baptismal/Ghana card
5. Medical Examination Form.
6. Four (4) recent passport size photographs
7. Personal record form.
8.UNDERTAKING FORM.
